Job Title: Supportive Services Coordinator

Location: Roxboro, NC

Salary: $20.00(hourly)

Schedule: 15 hours Per Week, Part-Time

Basic Functions:

  • Educate and inform residents about service availability, how to apply for and access services and benefits; and provide consumer advocacy;
  • Assist residents with forming or strengthening resident organizations; facilitate the formation of Tenant Associations within the property if residents are interested; Arrange or conduct resident leadership training sessions on how to structure meetings or write bylaws.
  • Help residents build informal supports with other residents and friends; arrange for senior companions or volunteer services.
  • Act as liaisons with management, community agencies and services, and advocate on residents behalf in groups, individually, formally and informally.
  • Monitor and establish links to community supports; educate service providers about resident needs and lack of resources, and encourage providers to utilize the Economies of Scaleconcept (serving more people at one consolidated price).

Specific Job Duties:

  • Provide general service management, which includes intake, education, and resident referrals (i.e., Meals-on-Wheels, transportation, home health aides, homemakers, financial assistance, counseling, preventive health screenings, and community benefits, etc.);
  • Fulfill professional educational requirements as outlined by HUD;
  • Assist residents with interpreting mail; completing forms; arranging utility, phone medical, and other payment schedules and visits; addressing errors or misunderstandings related to Social Security earnings, insurance billing, or death/survivor benefits; making funeral arrangements for loved ones; connecting with hospice, bereavement counseling, and general problem-solving;
  • Facilitate implementation and utilization of onsite or mobile health services and screenings;
  • Help residents obtain medical equipment and devices such as walkers, wheelchairs, talking books, large print telephones and other visual aids, grab bars, hearing impaired aids and devices, lever door handles, self-cleaning ovens, service pets, and emergency response systems;
  • Distribute and explain consumer materials from organizations such as State and

Area agencies on aging, the American Association of Retired Persons, the National Council on Aging, legal services; or medical insurances (such as Medicare and Medicaid);

  • Organize meetings to educate residents about housing development rules, regulations, and operations; and help managers and residents recognize and solve safety or accessibility problems;
  • Connect residents with community educational and recreational programs, and promote resident participation in local senior centers; link residents with HUDs Neighborhood Network Program, Senior Net, or other computer-oriented programs that help reduce isolation and increase independence;
  • Help residents work with health care providers to establish medication setup and reminder services; organize other reminder systems; negotiate quality discounts; identify and locate low-cost providers and products that can be delivered to residents or that offer transportation;
  • Develop a resource directory to include a listing of state and/or local services to families, children, elderly individuals, persons with disabilities, and emergency assistance resources (state and local government can also assist);
  • Other duties as assigned
