JOB DESCRIPTION:

SSW (Social Service Worker) Case Manager performs a variety of professional duties related to the administration of the New Choices Waiver, Alternatives, and Caregiver Support Programs. This includes intake, screening, eligibility, assessment, care plan development, service coordination, and maintenance of client files Services are provided in a community setting, which may include the client’s home, the home of a loved one or caregiver, assisted living facility, or other community setting. The position will also assist with information and referral, outreach, community events, and training.

EDUCATION AND EXPERIENCE:

Applicants should have a bachelor’s degree in behavioral or social sciences and be licensed in the State of Utah as a Social Service Worker (SSW) or equivalent case management certification from the National Academy of Certified Care Managers and a minimum of e year of work experience with the elderly population. Must have a valid driver’s license and the ability to travel to/from client’s place of residence and other locations as required to meet job duties.

Provide day to day management/oversight of Home and Community Based Services to include New Choices Waiver Medicaid, Alternatives and Caregiver Support Program to include eligibility, assessment, care plan and care coordination and maintenance of client files and case management activities.

Applicants must demonstrate excellent written and oral communication skills with an understanding of case management activities including assessment, care planning, and care coordination. Preference is given to individuals with one or more of the following: direct case management experience, experience working with long-term care supports and services including New Choices Waiver, and familiarity with human service resources in the Five County Area. Applicants must have experience and understanding of word processing and software applications, including Word, Excel, and web-based software programs. Applicant must be able to interface effectively with staff, professionals, and the public with knowledge of general office procedures and phone etiquette.

WORK-SITE:

Cedar City or St. George with Preference for individuals residing in Iron County/Cedar City Area- option to work at Five County AOG Office (Cedar City or St. George) or work remotely but must reside in the service area to meet Job requirements.

Job Type: Full-time

Pay: From $3,692.00 per month
