Social Services Director

The Citadel of Bourbonnais is currently seeking the services of a compassionate Social Services Director for the geriatric population to join our team.

Social Services Director Essential Job Functions:

• Identifies the social and emotional needs of individual residents.
• Assists in arranging services to meet the needs of residents.
• Participates in resident care planning.
• Ensures the social and emotional needs or reach resident are met.
• Provides advocacy between the resident, family, facility staff and the community.

Education and Experience Requirements:

• MSW, LCSW, or LSW preferred
• Previous experience in long-term care facility is preferred
• Working knowledge of Medicare and Medicaid is preferred
• Experience in creating quality assurance/quality improvement initiatives
• Strong communication and multi-tasking skills
• Compassionate and empathetic personality
