**This is for Burley, ID specifically. The office is located in Twin Falls

1. Commit to the Heritage culture. Be committed to providing excellent patient care while creating a culture of team work and compliance. Take ownership for your actions. Trust your home health team and always defend your team in a professional manor. Be positive and offer constructive suggestions when appropriate.

2. Be proactive and take responsibility for your part in making the team run smoothly. This includes following up on patient referrals and responding to admit texts in a timely manner; communicating with case manager, office staff and other disciplines regarding changes in patient status or plans of care; acquiring appropriate orders and/or prior authorization approval as needed; notifying team members of discharge; and following plan of care.
3. Function within your scope of practice. Know the rules or who to ask/where to find them. Familiarize yourself with the Agency policy and procedures or reference them when needed. Familiarize yourself with the State and Federal regulations that govern home care.
4. Be timely with charting. 7 days SRC’s/ROFU’s, 5 days DFA’s. 3 days daily visit notes. Orders are to be written and in the chart within the same time frame. This includes making corrections to charts that are returned for additional documentation. This will be found in your basket.
5. Good and frequent communication. This includes with all disciplines of a case management team, all office personnel, and also with patients/caregivers. It is expected for nurses to contact patients the day prior to admissions if at all possible and to notify disciplines of when your admission is. Check in frequently with team members to know what is happening with your pts progress.
6. Be proactive. This includes taking patient referrals, notifying office staff of discharges, volunteering to help cover patients for nurse on vacation or sick call.
7. Finding coverage. For all vacations. It is the case manager’s responsibility to find coverage for your patients. Full-time, part time, and PRN nurses may be used to help cover visits. If you are unable to find coverage, please notify your Clinical Director for assistance.
8. On-Call. It is expected that nurses are able to participate in covering weekend and weeknight call. No more than one (1) weekend a month will ever be required. Two (2) week nights per month will be the average for Monday-Thursday night call. Holiday call will also be covered primarily by full-time and part time nurses with PRN’s availability by choice of the PRN nurse.
9. Defend your home health team and trust each other.

10. When visiting local facilities, you should be signing in/out and completing a summary of your visit as an outside agency. If you do not know where to find this documentation, please ask.
11. If you are unable to make a regularly scheduled visit, it is up to you to attempt to find coverage using other team members and prn staff. If you are unable to find coverage, notify the Home Health Clinical Director to assist. Any uncovered visits that are missed need to have appropriate documentation in the form of a missed visit notification completed in the charting system.

12. Scheduled time off can be requested by filling out a request for time off and leaving it in the Clinical Director’s/ immediate supervisors in box or desk
