Job description
The ideal candidate for this position will have a background in social services and enjoy working in a team-oriented office. We are looking for an individual who is highly organized, professional and can work well under pressure. Our Case Manager will be responsible for researching long-term plans for clients and coordinating with other agencies to ensure that each case progresses smoothly. Have a knowledge of auto no fault and the recent reform . We deal with patients injuried in motor vehicle accidents who require treatment coordination.
Responsibilities:
- Accurately enter client data into a computer system.
- Establish and maintain accurate records for services to clients.
- Identify available community resources and refer people to appropriate service agencies.
- Provide assistance in obtaining public benefits, such as Medicaid and food stamps.
- Work with patients legal team to ensure they get medical records supporting the injury and monthly case managment reports.
- Advocate for clients by helping to resolve issues that arise with social service agencies or governmental offices.
- Help clients find adequate housing.
- Help clients pay their bills and access other financial assistance such as Social Security.
Job Type: Full-time
Pay: $40,000.00 - $80,000.00 per year
