Overview:
Maintains optimal health care for ICF individuals by administering medication and medical treatments, and monitoring health status. Coordinates the delivery of primary and specialized medical services under the direction of the RN, and serves as medical services advocate for each individual.
Responsibilities:
Primary duties and responsibilities include but not limited to:
  • Administers medication and treatments to residents by following doctor's orders to ensure individuals health needs are met and maintained. Consults with Registered Nurses and/or physicians regarding special medical needs or procedures.
  • Coordinates and ensures scheduling of individuals’ outside medical appointments by tracking monthly appointments, contacting providers, ensuring staffing coverage and accompanying residents and staff to ensure health needs are met in a timely manner.
  • Reviews individuals' health status by meeting daily with staff and supervisor. Communicates, both verbally and in writing, all medical information to staff and records such to ensure needs of individuals are met.
  • Ensures the individual’s health by monitoring status (i.e. takes temperature, blood samples), performing tests/procedures, and following-up to provide residents with maximum health.
  • Participates as a member of the inter-disciplinary team by attending meetings and following through on staff recommendations to ensure individual’s medical needs are met.
  • Ensures adequate supply of medical supplies and medications are on hand by filling out necessary medical forms, monitoring and ordering current supplies to ensure adequate supplies and medications are available for individuals.
  • Utilizes medical database to ensure timely, comprehensive and accurate medical reports.
  • Completes monthly vital signs and weight monitoring of individuals and reports concerns to RN and MD.
  • Performs additional related duties as requested by supervisor.
  • Ensures that work-related safety procedures are in place and are being adhered to by staff observance of an unsafe act or a potentially hazardous work environment must be immediately reported to supervisor and/or safety committee.
  • This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services and therefore, is subject to successful completion of fingerprint/criminal background check.
  • Reports to Justice Center as a mandated reporter when there is reasonable cause to suspect abuse or neglect or a potentially hazardous work environment. Additionally, follows all agency incident policy and procedures.
  • Adheres to all regulations, agency policy and procedures, the employee handbook and agency and program training requirements.
  • Arrives to work by start of scheduled shift, adheres to agency policy and procedure in regard to call-offs and overtime to ensure quality support is provided to people each day.
  • Performs other related duties as instructed by supervisor.
Qualifications:
  • Must be a NYS licensed practical nurse.
  • CPR training/certification, fire safety training, and medication knowledge is required.
  • Six months of hospital or long-term care experience, preferably working with the ID/DD population.
