We have an URGENT need for dedicated, caring Home Health Aides.

Requirements to apply:

• Valid NYS Home Health Aide certificate
• Complete physical exam including PPD dated within the last 12 months (we will provide form for your MD)
• Must include chest x-ray positive dated within past year if PPD is positive
• Must include Rubella and Rubeola titers (attach lab results)
• Must include drug screen from past 12 months
• Two reference letters with telephone numbers
• Two pieces of current identification, one with photo
• Drivers License a plus
• Covid Vaccine card
