Full Time HIV Medical Case Manager.

The Medical Case Manager engages clients of FSABC’s HIV/AIDS Program in designing and following a goal-oriented plan that responds to his/her health (physical and emotional), social, and environmental needs identified through formal assessment and re-assessment interviews. The Medical Case Manager empowers the client to maximize use of personal and informal resources, links them to community and formal resources as needed, and assures that resources are the least restrictive and least intrusive possible. The Medical Case Manager acts to ensure that the client obtains timely access to needed services, appropriate levels of care, and continuity of care with the overarching goals being to improve their health care outcomes and promote adherence to their HIV treatment.

RESPONSIBILITIES:

  • 1. Receive referrals from AACO CSU as assigned by HAP Coordinator and enter client into CAREWare database.

2. Conduct face-to-face assessments with clients to identify areas of need and to develop Service Care Plans. The Medical Case Manager has knowledge and interviewing skills needed to differentiate relevant historical data, to discern meaningful patterns in client behaviors, and to exercise sound judgment in interpretation of data.

3. Assists clients with implementation of Service Plans by effecting linkages between service systems, helping clients to access needed services, coordinating multiple services, monitoring and following-up with the client and service providers, and by case advocacy when necessary. The Medical Case Manager has excellent knowledge of community resources and sound judgment in prioritizing needs and formulating activities to meet those needs. The Medical Case manager will maintain regular, periodic contact with client to check on follow through with referrals (linkages) and to assess their health status

4. Transport clients to needed appointments when necessary

5. During times of crisis, refer clients to appropriate resources and provide support.

6. Completes and maintains orderly, legible, timely records of services and other documents in narrative or data formats as required by the agency or its contracts.

7. Pursues continuing education to maintain and up-grade social service knowledge and practice skills. Adheres to requirements for continuing education set forth by AACO. Uses supervision to identify and address professional development needs. Specially attends to on-going development of sensitivity to diverse cultural groups, particularly groups served by this position.

Education: Bachelor of Social Work from a school accredited by the Council on Social Work Education or Bachelor's degree in a closely related health or human service profession from an accredited college or university.

Experience: Two or more years of successful employment in direct service practice at the Bachelor's level and including case management experience specifically relevant to this position and serving clients with characteristics similar to those of the specific population targeted by the agency program, project, or service covered by this position. Comprehensive knowledge of community-wide health and human service systems and facility in identifying resources appropriate to the kinds of problems commonly encountered by the client population served by this position.

Candidate must be able to pass the following:

o Drug Screen

o FBI/Fingerprinting Clearance

o Pennsylvania Criminal Clearance

o Pennsylvania Child Abuse Clearance

Pay: $38,000.00 - $41,000.00 per year
