As an Agrace CNA you will collaborate with patients, families, Registered Nurses, physicians, facility partners, and Agrace care teams.
Responsibilities:
Our CNAs are responsible for visiting patients rooms and following the plan of care as identified by the Registered Nurse.
- Assistance with ADLs
- Position adjustments
- Light homemaker help
- Communicating change in condition

- Current, unrestricted Wisconsin Nursing Assistant Certification
- One-year personal care experience; hospice or home health experience preferred.
