Join us as the Care Coordinator at our Fairlawn / Akron, Ohio location as we expand mental health services to children & adolescents.
Position: Care Coordinator
Beyond Healthcare is a new mental health agency that focuses on the child and family healing together. We are passionate about bringing the humanity back into healthcare and connecting people to a vibrant community where they belong, find their purpose, and heal.
Due to growth we are hiring a care coordinator to help with patient care needs at our mental health facility for adolescents. This position will be focused on expansion of our group programs, helping with group facilitation and support for our teen PHP/IOP groups.
Schedule: Full-Time, Monday-Friday 8:30am-5:00pm
Main Responsibilities:
- Participates in the development and coordination of the treatment plan
- Coordinates crisis intervention services
- Care coordination with other community partners, referral agencies, and invested parties
- Assist with co-facilitation of groups including facilitation of activities, coordination of transportation or patient food needs
- Serves as an advocate for clients and communicates with significant supports of client
- Confers with supervisor and other team members regarding client needs
- Assists clients in tracking and keeping appointments at Beyond Healthcare and other specialists
- May be after hours “on-call”
- Completes required paperwork in accordance with timeliness and standards required by Beyond Healthcare
- Must be eligible to drive a company vehicle as defined by Beyond Healthcare’s commercial insurance carrier and assist with client transportation if necessary

Experience Required:
- At least 2 years experience working with teens in a social services environment preferred
- Qualified Mental Health Professional preferred (QMHS)
- High School Diploma or GED required
