Meadowbrook Terrace– LPN – Full Time Day Shift (Req 6436)

Internal Posting Dates: April 11, 2023- April 15, 2023

Plans and implements the medication management and coordinates nursing care of residents within the assisted living setting with the supervision of a Registered Nurse. Responsible for ensuring continuity of care of the residents between shifts by providing direct care as well as supervising the care given by PCA/HHAs and supportive staff members. Participates in resident and family teaching. Maintains role as resident advocate with a focus on the facility’s mission. Responsible for management of assigned school based practices, their staff and office operations.

JSUMMAR

Requirements:

• Valid NYS LPN Licensure - required
• Current AHA BLS Certification
• Current AHA First Aide Certification
• Choking Rescue Training (Heimlich Maneuver)
• High School Diploma or GED
• Basic computer knowledge
• Must have the ability to read, write and follow oral/written instructions in English, document care given, record messages, keep simple records and speak the predominant language of the resident
