POSITION SUMMARY
Responsible for the support and direction of the Social Work department through; planning, organizing and implementing social work services that meet the needs of AtlantiCare LIFE Connection participants and families. Responsibilities include but are not limited to: development of goals, outcomes and objectives for the department; assessment of staffing needs; team building; participation in quality improvement initiatives; providing direct participant care via assessments and treatment; teaching and counseling to participants as needed.
Development and oversight of Behavioral Wellness services programming for AtlantiCare LIFE Connection participants. Performs in person assessments for enrollment of potential participants to obtain a complete psycho-social history, which may include descriptions of cognitive status, social supports, family dynamics, mental health and substance dependency as needed. Coordinates with the interdisciplinary team to develop a comprehensive care plan relative to the behavioral health needs of each participant.
Works in conjunction with other leadership team members to ensure processes are established to maintain Medicaid eligibility for LIFE Connection Participants. May serve as Interdisciplinary team and care planning facilitator. Performs NJ Choice level of care assessments and training as needed. Develops and organizes educational programming, including but not limited to CPI and substance use disorder programs, for the LIFE Connection staff in collaboration with the appropriate leader. Participates in administrative on call rotation

QUALIFICATIONS
EDUCATION: Master's degree from an accredited school of social work required.
LICENSE/CERTIFICATION: Current New Jersey state licensure LCSW or LPC required. Valid NJ Driver's License, Registration and Insurance are required.
EXPERIENCE: Minimum 3-5 year's health care knowledge with combined clinical and management experience in social work experience required. Minimum one 1 experience, working with the elderly required. Good public speaking skills with all size groups required.
Effective verbal and written communication skills, required. Excellent knowledge of community resources and Medicaid processes required. 5 years' experience working on an interdisciplinary team in a hospital, nursing home or community-based setting preferred. NJ Choice certified trainer and assessor preferred. Psychiatric counseling experience with senior population preferred.
Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.
PERFORMANCE EXPECTATIONS
Demonstrates the competencies as established on the Assessment and Evaluation Tool for this position.
WORK ENVIRONMENT
This positon must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants' care. The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled temperatures.
This position also requires corrected vision and hearing to normal range. Incumbents must be able to communicate with all staff, vendors, and clients. Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment. This position also requires lifting/carrying up to 65lbs.and pushing or pulling 150lbs.using appropriate body mechanics and equipment, with reasonable accommodation if
needed.

This positions reports to department leadership.
