Looking for a Part Time Nurse Practitioner (NP) for Home Health. Flexible Schedule job in Clark County, AR. The Part Time Nurse Practitioner (NP) will conduct home health assessments.

The average home health risk assessment will take about 45 minutes and the nature of the visit may vary and could include an annual health assessment or post discharge visit. The Part Time Nurse Practitioner (NP) will assess for gaps in care for preventative measures and provide digital recommendations to the primary care physician for gap closure.

The home health assessment will be conducted on Medicare Advantage Plan members residing in the Clark County, AR area.

Requirements

Responsibilities:

• Conduct annual home health risk assessments for a Medicare Advantage Health Plan

Evaluate prior and current medical history, medical conditions and medication historyConduct point of care testing including fingerstick and urine testingProvide education on medical conditions and medicationsIdentify any gaps in care and provide recommendations for resolutionNo creation of treatment plans or prescribing of medications is requiredWork Hours:Flexible shifts between Monday and SundayCreate your own scheduleMinimum weekly or bi-weekly hourly commitmentPreferred Experience:

• 1 year of current NP experience is preferred

Previous in home care experience is preferred, but not requiredPreferred Qualifications:Nurse Practitioner License in AR

Current CPR certification (BLS)Board Certification through AANP or ANCCEducation Requirements:MSN degree from an accredited school of nursingIndustry: Healthcare
