Responsibilities:

The Central Community Health Partnership (CCHP) is a collaboration of five premier human service organizations in Central Massachusetts that has been selected to provide Behavioral Health Community Partner services and Long-term Services and Supports (LTSS) through the MassHealth program. For people with Behavioral Health Challenges finding the right combination of services and knowing how to access them can seem to be an overwhelming task. Central CHP stands ready to help MassHealth members with complex needs navigate the confusing array of choices. The vision is integrated care that ensures that every member receives the right care at the right time – making good health and wellness a reality, not a dream. The four organizations – AdCare, LUK, Inc., Open Sky and Venture Community Services – bring a wealth of experience helping people in Central Massachusetts reach their health and life goals.


The Registered Nurse will assist the CCHP’s Behavioral Health Community Partners Program to effectively manage the day-to-day medical issues that arise while providing care coordination and care management services to individuals served. The RN will be responsible for the completion of a comprehensive assessment for each participant’s health needs and will support individuals served in gaining access to health care while connecting them to community resources as appropriate to meet their individualized needs.


Other key responsibilities include:

  • Review clinical records on a regular basis to ensure adequate services and supports are provided, referrals are obtained as needed, and that health screenings and diagnostic testing is done as indicated.
  • Assist in the collection of outcomes information, annual analysis and other reporting and initiatives as required.
  • Collaborate with providers, individuals, care coordinators, medical and behavioral providers as necessary.
  • Complete all required documentation in a timely manner.
Qualifications:
  • Licensed registered nurse (RN) in the state of MA.
  • At minimum, 2 years’ experience working with adults with serious and persistent mental illness and substance use disorders.
  • Must be able to utilize local resources, navigating health, behavioral health, and/or substance abuse treatment systems.
  • Excellent communication skills, documentation, time management, and organizational skills.
