The Quality Improvement Specialist provides assistance in assuring compliance with CMS Quality measures and facilitation of performance improvement initiatives through collaboration with the clinical quality improvement advisor across inter-disciplinary teams and departments.

Quality and Process Improvement 70%

  • Supports quality of care compliance with CMS Quality Measures through vigilant data review and ensuring adherence to timeline requirements.
  • Reviews and monitors outcomes that promote compliance with the Quality Improvement Program.
  • Assists in process improvement activities in response to compliance audits, satisfaction surveys and identified variances using LEAN methodology.
  • Demonstrates ability to track and trend quality initiatives and regulatory requirements and assist with data analysis.
  • Participates in programmatic committees as determined.
  • Provides support with Patient Centered Medical Home (PCMH) readiness and accreditation activities, as needed.
  • Supports Infection Control surveillance activities in collaboration with the clinical quality improvement advisor through diligent data management and review.
  • Develops and performs auditing activities related to quality measures, surveys, and process improvement activities as appropriate to support monitoring and adherence to quality program standards.
  • Assists and participates in quality training and other initiatives as deemed appropriate.
  • Reviews, tracks and trends incident reports and other quality measure regulatory requirements for completeness and facilitates identification of areas of opportunity from a quality and safety perspective in collaboration with the clinical quality improvement advisor.
  • Prepares for and actively participates in all surveys/monitoring audits and ensures that InnovAge and its affiliates are survey-ready.
  • Participates in CMS and State reporting and facilitates work groups as appropriate within the PACE centers and operational or compliance leadership.

Administration 30%

  • Assists with development of Policies and Procedures as appropriate.
  • Collaborates with the Management Team to ensure compliance with Quality related Policies and Procedures.
  • Applies vigilance for responsibility of data entry to Health Plan Management System (HPMS).
  • Assists in all Quality meeting preparation activities to promote efficiencies.
  • Demonstrates a strong ability to maintain and aggregate data as needed.

REQUIRED

  • Bachelors degree in related field or an equivalent combination of experience and education.
  • Minimum of three years working in Quality Assurance in healthcare related environment.

PREFERRED

  • Demonstrated success with Performance Improvement Initiative.
  • LEAN or Six Sigma
