SUMMARY

Qualified Developmental Disability Professional (QDDP) means a person who possesses at least one year of documented experience working directly with individuals who has an developmental disability and has a registered nurse licensed in Virginia.

Immediate position for a QDDP with RN License to develop/update ISP Plan V and related assessments for DD waiver recipients in Northern Virginia.

DETAILED

• Qualified Developmental Disabilities Professional (QDDP) needed to develop and update the annual Person Centered Individual Service Plan (ISP) Plan V (Plan for Supports) for each individual supported
• Ensure the implementation of the particular requirements of the ISP
• Complete semi-annual documentation regarding individuals’ satisfaction with services.
• Complete the Direct Support Professional (DSP) Assurance and DSP Competencies Checklist
• Complete annual supervisor competencies checklist.
• Knowledge of DBHDS, Office of Human Rights, WAMS, CHRIS and Medicaid Waiver Guidelines/Regulations

EDUCATION and LICENSE

The QDDP must possess at least one year of documented experience working directly with individuals who have developmental disability and a registered nurse licensed in Virginia
