Join our Team! We have an exciting new opportunity for a Clinician in our OnTrack Program in Yonkers, NY, New York, Hybrid. Base Annual Salary Range: $50,000-$77,000.

Summary:

OnTrack is an innovative treatment program that serves adolescents and young adults (16-30 years old) who are within two years of experiencing the first onset of psychosis. Our mission is to keep young people on track with their goals for school, work, and personal relationships. Equity, inclusion, rapid access, and self-determination are at the core of everything we do. Our team provides flexible, supportive, and collaborative multidisciplinary individualized care.

The Primary Clinician will be part of a small multidisciplinary team providing coordinated and assertive treatment. Training for this role is provided through the statewide OnTrackNY Initiative at the Center for Practice Innovations at Columbia Psychiatry and New York State Research Foundation for Mental Hygiene.

SPECIFIC DUTIES and RESPONSIBILITIES:

  • Serve as the point person for the participant and their family.
  • Provide psychoeducation regarding first-episode psychosis (FEP) and the goals of the OnTrackNY program.
  • Provide supportive therapy
  • Provide behavioral interventions and take an especially active role in the family support domain.
  • Conduct a clinical assessment to determine the participant’s level of social functioning and substance use via a shared decision-making (SDM) framework.
  • Provide Social Skills Training, Substance Abuse Treatment, Behavioral Activation, Coping Skills Training, and Psycho-education.
  • Connect participants and families with the services and support their need.
  • Develop and modify treatment using a shared decision-making process with clients.
  • Provide services in the community (off-site) as needed based on participant and family preferences.
  • Conduct needs assessments.
  • Engage in safety planning and wellness management planning with participants.
  • Work with participants and family for the transition from the team to other services and supports, and ensure stable links at discharge
  • Attend weekly team meetings.
  • Other designated and related duties.

REQUIRED KNOWLEDGE and SKILLS:

  • Experience in an Article 31 Mental Health Clinic preferred.
  • Experience with outreach to underserved populations and direct service to people with serious and persistent mental illness
  • Knowledge of applicable city, state, and federal guidelines and laws affecting mental health treatment, services, and reporting procedures.
  • High degree of computer literacy
  • Familiarity with recovery-oriented, evidence-based clinical practice preferred.
  • Experience with dually diagnosed individuals preferred.
  • Demonstrated experience with diverse populations preferred.
  • Understanding the impact of culture on mental health practices
  • Familiarity with and ability to use an Electronic Health Record system.
  • Demonstrated commitment to diversity, equity, inclusion, and cultural humility.

QUALIFICATIONS:

  • New York State licenses: LMSW, LMHC, Psychologist or LCSW
  • legally eligible to work in the United States
  • Reliable and able to work independently and understand the importance of maintaining confidentiality.
  • Bi-lingual Spanish preferred
  • Must have and maintain a valid Driver’s License with a driving record that is satisfactory to our insurance carrier.
  • Must have and maintain a Defensive Driving certification.
  • Must be cleared NYS Justice Center through fingerprinting for a Criminal History Records search.
  • Must be cleared by the Justice Center for the Protection of People with Special Needs Staff Exclusion List (SEL) and the New York State Medicaid Exclusion List
  • Must be and maintain clearance by the DCJS New York State Sex Offender Registry.
  • Must be cleared by the NYS Office of Children and Family Services (OCFS) for instances of child abuse and/or neglect.
  • Must be legally eligible to work in the United States
