Elite Home Health & Hospice is seeking a Nurse Practitioner for the Palliative Services program. We currently support our entire service area with palliative care and are rapidly growing. We are hoping to add an additional nurse practitioner full-time or 2 part-time practitioners to support this growth.

An average day, includes 4-5 appointments with patients, fluctuating based on evaluations versus routine visits. Each patient is typically seen once per month depending on clinical needs. The nurse practitioner is supported by the medical assistant/coordinator who gather information, builds the chart, completes preliminary paperwork, able to collect labs if needed, and supports the nurse practitioner to allow for streamlined visits and documentation.

We utilize E-Med Practices as our medical record system and provide Dragon for documentation support. We support continuity of care and support hospice for patients who have bridged and collaborate with home health for patients needing rehabilitative services.
