Overview:
Western Michigan University Homer Stryker M.D. School of Medicine (WMed) has an opportunity for a Street Medicine Nurse Navigator. Street Medicine Kalamazoo (SMKzoo) provides medical care to unhoused individuals on the streets, in encampments, and shelters where they are temporarily housed. The initiative was started in January 2021 by medical students, resident physicians, and attending physicians from WMed. SMKzoo’s model of care helps overcome systemic barriers by taking medical care directly to the people and establishing a relationship of trust and mutual respect with the unhoused community while training the next generation of highly skilled and compassionate physicians.

SMKzoo is seeking a Registered Nurse to work with the team to provide primary care to unhoused and precariously housed individuals living in the city of Kalamazoo. The Nurse Navigator will work closely with the physician Director and Associate Director. The ideal candidate must have a commitment to providing high-quality care to complex patients, dedication to working and developing a collaborative team model of care, and experience working or volunteering with the unhoused population. They should be open to learning the principles of harm reduction and an interest in women’s reproductive rights
Responsibilities:
  • Provides outreach, information and referral, care coordination, and case management services as appropriate for Street Medicine.
  • Conducts follow-up with patients via phone calls, encampment visits, and visits to other settings where patients can be found.
  • Completes initial assessment and triaging of unhoused and precariously housed patients referred to Street Medicine.
  • Assists with hospital discharge planning for unhoused and precariously housed individuals transitioning from the hospital back to the community while ensuring a safe discharge with an after care plan.
  • Provides health screening for various medical conditions, health education, medication management and monitoring.
  • Performs continued assessment of client health status including but not limited to wound care, immunization, specimen collection, and physical examinations.
  • Administers medications and injections as prescribed by a physician.
  • Implements plans of care for both urgent and primary care in collaboration with the care team and within the scope of a Registered Nurse.
  • Uses the electronic health record to gather patient information; documents appropriately and maintains medical records; schedules patient appointments.
  • Provides patient care in a manner that is both non-discriminatory and non-judgmental.
  • Works cooperativey with other clinical personnel and exhibits excellent working relations with patients, visitors, and staff, effectively communicating SMKzoo’s mission.
  • Acts as a patient advocate and liaison between the patient and community agencies (e.g. Department of Human Services, Integrated Services of Kalamazoo, hospitals/clinics) and ensures that patients’ needs are met with rights maintained; consults and collaborates with community providers to ensure continuity of care.
  • Availability to work occasional overtime on weekends or evenings to complete time sensitive projects.
  • Acquires knowledge for new technology and policy/procedure revisions.
  • All other duties as assigned.
Qualifications:
EDUCATION AND/OR EXPERIENCE:
  • Associate’s degree in nursing, Bachelors education preferred.
  • At least one year of experience working with the unhoused population preferred.
  • At least three years experience working as a Registered Nurse preferred.
LICENSES, REGISTRATIONS, CERTIFICATIONS:
  • Active RN license in the State of Michigan.
  • Current BLS certification.
OTHER SKILLS AND ABILITIES:
  • Knowledge and proficiency in working with the Electronic Health Record system (Epic and Cerner).
  • Knowledge and proficiency in word processing and file management using Microsoft Office, Outlook and other software as required by the department.
  • Knowledge and proficiency in grammar and spelling skills.
  • Ability to effectively communicate with others verbally and in writing.
  • Ability to interpret, comprehend, and transmit complicated and detailed instructions accurately.
  • Demonstrates the ability to recognize priorities in organization of work flow.
  • Able to perform duties independently, with a minimal need for direct supervision.
