Transcribes medical and psychiatric dictation using digital dictation system. Reviews and edits transcribed reports or dictated material for spelling, grammar, clarity, consistency and proper medical terminology prior to entry into the patient electronic record.
Reviews, analyzes, and prepares patient documents for scanning into the patient electronic record. Other duties as assigned.

Individual must be available to work hours other than 8:00 - 5:00 pm and participate with weekend and holiday on-call rotation.

Will be under direct supervision by the HIM Team Lead.

Compliance with HHS Immunization Policy required: serologic evidence of immunity to, or documented evidence of vaccination against, measles, mumps, rubella, varicella (chickenpox), Tdap, annual vaccination against influenza (and hepatitis B if applicable).


Essential Job Functions:
Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.

Shall transcribe/enter into CWS all MD History and Physicals, Psychiatric Evaluations, AIMS
Assessments, Columbia Suicide Risk Assessments, Physician Progress Notes, Physician Discharge Order Notes, in accordance with organization and department policy and procedures. Will be responsible for ensuring the editing and proof-reading of all transcribed reports/CWS entries before e-filing in the patient record. Will also maintain a current, complete and accurate log of all completed assessments

Will monitor CWS Concurrent Audit Reports and provide timely notification to physicians and staff to ensure core assessments are completed and finalized within the specified timeframes.

Will retrieve, disassemble and analyze discharge records for completion and accuracy.

Prepares paper records/documents for scanning into the electronic health record. Creates digital images of paper records/documents.

Performs other duties as assigned.

Other duties as assigned by HIM supervisory and management team.

Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.



Knowledge Skills Abilities:
This position requires medical transcription work experience as well as excellent verbal and written communication skills in order to effectively interact with various discipline levels and patients. Employee must be able to work independently and productively with all levels of staff. Skilled in the use of personal computers and software applications such as Microsoft Word, Excel and Outlook. Must have experience in the use of electronic medical records. Must ensure that no information, facts, or comments learned in the performance of duty are discussed in order to protect the confidentiality of the patients served and/or Rio Grande State Center. Ability to type at least 50 words per minute. Knowledge of the maintenance and review of medical records for timeliness, completeness, accuracy and compliance. Must be able to transcribe physician dictation with minimal error. Job requires being detail oriented and thorough in completing work tasks.

Initial Selection Criteria:
Must be a graduate from a standard senior high school. Must be a graduate of an accredited Medical Information Specialist / Medical Office certification program or Health Information Technology program.
Must have medical records department, medical office or medical unit clerk work related experience.
Must have knowledge of spelling and use of medical terminology as evidenced by completed skills test.
Must be skilled in the use of Microsoft Office applications to include Word, Excel and Outlook.
Must be able to type a minimum of 40 wpm.

Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aid, Medication Aide and Employee Misconduct, and HHS OIG list of Excluded Individuals/Entities. The salary offered will follow HHS starting salary guidelines which are typically made at or near the posted minimum of the salary range.

Must be able to apply attention to detail to ensure accuracy. Has knowledge of medical office/hospital terminology. Knowledge in the use and maintenance of databases. Ability to cross train in various areas of the health information management department. Must be able to protect the confidentiality and privacy of the clients and patients served by ensuring that no information, facts, or comments learned in the performance of duty are discussed. Employee must be able to participate in weekend and holiday on/call rotations when needed.
