Esse Health is currently seeking a full-time Manager of Clinical Support.

Summary:
The Manager of Clinical Support leads a team of health care professionals providing care to Esse Health patients in the home, telephonically and remotely in Missouri and Illinois. He/she will work closely with physicians and office management within our 45+ clinical locations.

This individual focuses the team on using population health strategies to create improved processes, implement change and maximize patient care. Essential to the role is the ability to collaborate and participate in care coordination activities that meet the clinical and social needs of the patient.

In developing best practices for the teams, this individual will collaborate with the Manager of Care Transitions and the VP of Clinical Programs.

Qualifications:

  • Education: Master of Science in Nursing (MSN)
  • Licensure/Certification: Licensed in MO and IL, CCM certification or APN preferred
  • Years of Experience: Minimum of 5 years inpatient or outpatient clinical experience and at least 5 years in a management or leadership role

Duties & Responsibilities:

  • Direct supervision and management of a team of multi-site health care professionals in the home and Central Business Office.
  • Collaborate with team on plans focused on directing and coordinating the referrals sent to the care management team.
  • Determine clinical metrics, performance standards, expectations, and policies and procedures with Vice President of Clinical Programs and the Manager of Care Transitions.
  • Analyze team performance, success and opportunities using care management reports, quality metric data, provider and operational reports.
  • Initiate process improvement activities to integrate best practices and standards of care efficiently and effectively.
  • Develop and implement training and educational programs for defined staff.
  • Establish and maintain relationships with home health agencies, vendors, community partners, behavioral health collaborators, payers and social service resources.
  • Facilitate all internal home health, social worker and office care management team calls and meetings.
  • Attend all scheduled leadership and care management meetings. Develop monthly educational trainings and/or schedule the presentations for the department.
  • Coordinate with other leaders on the goals, expectations and activities designed to enhance teamwork and build a unified team.
  • Assist Primary Care Provider (PCP) and office manager with challenging and complicated care management referrals. Facilitate the coordination and communication with the appropriate team member.
  • Participate in creating and implementing patient focused programs and interventions. Responsible for educational activities for office health coaches.
  • Responsible for payroll, schedule, PTO, performance reviews and employee issues.
  • Complete all other duties as assigned.

  • Experience managing a diverse team of health care providers who work in the home and office.
  • Able to define problems, formulate solutions, make recommendations, and initiate corrective actions leading to positive outcomes.
  • Strong leadership and teaching skills to develop cohesive, creative, and successful team members.
  • Knowledge in program development, implementation, and process improvement.
  • Analytical skills to create and use reports and data to set goals, support and improve qualify initiatives.
  • Demonstrated ability to meet established targets and deadlines.
  • Excellent interpersonal and communication skills to initiate and maintain collaborative relationships.
  • Strong organizational skills to perform a variety of tasks independently, timely and effectively.
