Position Summary:

The Manager of Care Support Nursing is responsible for providing programmatic and clinical direction, oversight, and guidance to a team of Care Support Nurses who serve diverse populations of patients with complex medical needs across clinic sites. The Manager is responsible for monitoring and evaluating team, program and contract performance as defined by quality of care, compliance, financial indicators and sustainability metrics and adjusting programs, services and staffing accordingly. The Manager initiates, oversees and implements continuous development and improvements to workflows, programs and systems in conjunction with other Care Support programs and leadership at HealthPoint. Compensation is dependent on skills and experience.

Your contribution to the team includes:

  • In collaboration with Director of Care Support, set direction and strategy for Care Support Nursing services, scope, programs, care model and staffing. Implement and maintain a comprehensive care management model focused on ensuring whole person care within an integrated primary care team.
  • Collaborate closely with Director of Care Support and other Care Support team members to advance the care model, team integration and high-quality patient support and care.
  • Nurture a positive work environment that fosters growth, development, and sustainability of talented staff.
  • Responsible for hiring, performance management, talent development, employee relations, recognition and training of staff by providing effective leadership. Ensure all work is completed in support and enhancement of the objectives and goals of the organization.
  • Continuously evaluate and assess program strengths and opportunities. Make recommendations and devise solutions to complex programmatic, contractual, and funding situations.
  • In conjunction with clinic leadership, directly collaborate with Care Support Social Work leadership to oversee interdisciplinary care teams and devise strategic workflow, care model or program enhancements.
  • In collaboration with Director of Care Support, accountable for financial sustainability of assigned programs and services.
  • Identify barriers and potential solutions to program and staff development across sites. Advocate for programmatic and team integration and sustainability including important recruitment or retention strategies.
  • Articulate and integrate the Care Support Nursing role into the care team to provide clinical insights and direction from a nursing perspective
  • Maintain knowledge of current best practices and programs, develop innovative approaches to care provision accordingly.
  • Cultivate diverse care teams to launch and build new programs or projects as needed.
  • Partner with internal departments to streamline and optimize internal processes to continually improve workflows, system functionality, personnel oversight strategies, electronic resources, communication, and administrative functions.
  • Seek ways to standardize workflows, intake and assessment tools, and other program elements to maximize care team expertise, meet or exceed quality metrics, and maintain high levels of patient and staff satisfaction.
  • Build and strengthen community partnerships to advance health equity and enhance access and services for patients receiving Care Support services, acting as liaison and ambassador for Care Support and HealthPoint as indicated.
  • Partner with external teams and organizations to strengthen core services and workflows of Care Support such as transitions of care, and other processes impacting social determinants of health.
  • Play a key role in alternative funding source procurement and sustainability such as grant writing contributions, funder reporting and project implementation.
  • Partner with marketing and business analytics to promote programs and services.
  • Continuously align work priorities with organizational goals, adjusting strategies and programs accordingly.
  • Adeptly navigate sensitive conversations with staff and teams including development plans, coaching, work performance and productivity with emotional intelligence that nurtures self-awareness and growth within team members.
  • Identify, advocate for, develop and implement needed reports, data analytics solutions, documentation improvements and other program enhancements as needed.
  • Provide Care Support Nursing and care coordination expertise for complex patient or care team scenario consultation as needed.
  • Provide clinical oversight and evaluation of various screening tools and Care Support Nursing interventions such as: PRAPARE or other Social Determinant of Health screening, KATZ, GAD7, PHQ9, Motivational Interviewing, care management assessments, and safety planning with patients. Supplement learning and expertise of all reporting staff and leadership within discipline, implementing changes when indicated.
  • Collect, analyze, and present data regarding the performance of designated Care Support Nursing programs including enrollment, capacity metrics and financial status to inform and expand services as needed.
  • Run and maintain regular reports and utilize electronic resources to meet program, budget, and contractual requirements. Present data and information to internal or external groups as needed.
  • Contribute to implementation and refinement of Health Home and other program procedures, workflows, and measures to align with contract deliverables and business needs. Take lead role as indicated.
  • Provide appropriate orientation and program training for assigned staff. Provide additional clinical training and support as needed.
  • Ensure effective communication exists between staff, care team members, coworkers, patients, and support systems.
  • Provide coaching and guidance to maintain professional relationships and boundaries as staff support patients, families or caregivers with empathy, compassion and cultural congruence while maintaining respect for confidentiality, privacy, and mandated reporting.
  • Provide effective public speaking, group presentations and/or team facilitation as needed.
  • In collaboration with Director, responsible for annual creation and execution of assigned budgets.
  • Meet with team members, clinics, or patients in a variety of settings, including clinic, community, office, or patient’s home as needed.
  • Conduct department operations using HealthPoint’s LEAN Management System methodologies to identify waste, determine system improvements, and implement changes through a structured process.
  • Provide updates to key stakeholders regarding progress toward goals.
  • Provide direct Care Support Nursing care to patients
  • Meet with external agencies and give presentations.

Must have’s you’ll need to be successful:

  • Bachelor’s degree in Nursing from a four-year college or university and seven (7) years related experience in a clinic or hospital setting, working with vulnerable populations, behavioral health or community health, of which three (3) years is care management or coordination experience and of which one (1) year is supervisory experience or equivalent combination of education and experience required.
  • Master’s in Nursing, relevant certification and/or bilingual language proficiency strongly preferred.
  • Current Unencumbered Washington State Registered Nurse license required and must be maintained.
  • Valid Washington State Driver’s License with an acceptable driving record determined by HealthPoint’s insurance carrier.
  • Intermediate level of Word, Excel, Outlook and Electronic Health Record (EHR) required.
  • Willing to learn OTTO or any other electronic platform.


Proof of vaccination for COVID-19 is required, prior to start. All new employees are also required to show proof of immunizations and/or immunity to MMR (measles, mumps, rubella), Varicella, annual Influenza and TB QuantiFERON Gold Titer. Additionally, if you work in a HealthPoint clinic, Tdap (within last 10 years) is required. Hepatitis B. is required for clinical employees with potential exposure to blood/blood products. All immunizations are a condition of employment. Upon hire, employees must provide proof of their immunizations and/or immune titer results prior to starting or no later than their fifth (5) business day of employment.
