Fairview Commons Nursing and Rehabilitation Center is a 146-bed Skilled Nursing Facility located in the Berkshires and provides both short-term rehabilitation stays and long-term care services. The Licensed Social Worker will be working with the Interdisciplinary Clinical Team to ensure that the resident’s physical, mental, and psychosocial needs are met. The Licensed Social Worker’s primary responsibilities and qualifications are as follows but are not limited to:

Responsibilities:

• Assists short-term and long-term residents and their families in adjusting to their illness, by dealing with life changes, grief, loss, placement, and depression through counseling and developing resident-centered care plans with other members of the Interdisciplinary care team.
• Assists with advanced directive planning in collaboration with nursing and medical services.
• Responds to resident and family needs by advocating on behalf of the resident any wishes that are presented. Attends appropriate facility meetings on behalf of the residents such as Care plan meetings, Welcome meetings, Medicare meetings, etc.
• Completes documentation per federal and state guidelines and sends required documentation to appropriate agencies, discharge locations, and care teams.
• Educates residents and families regarding financial resources.
• Assists with the orientation of new employees as it relates to resident rights.
• Demonstrates awareness of respect for, and attention to the diversity of the people with whom they interact (persons served, personnel, families/caregivers, and other stakeholders) that is reflected in attitudes, behavior, and services.

Qualification

• Must be licensed as a Social Worker in the Commonwealth of Massachusetts.
• Bachelor's Degree in Social Work or Human Services, LMSW is highly preferred.
• Nursing Home experience is preferable but will consider previous experience working in other healthcare settings.
• Prior Social Work management experience in nursing homes or similar settings will be considered for the lead/Director’s position.

Job Type: Full-time

Pay: $26.00 - $34.00 per hour
