POSITION: Independent Living Specialist

Position availability:

• Mon- Wed & e/o 8a-4p
• Thur, Fri & e/o wknd 8a-4p
• Mon - Wed & e/o 4-10pm
• Thur, Fri & e/o wknd 4p-10p

LOCATION: Eagan/Apple Valley Area

DESCRIPTION: This position is a direct care position working directly with the individuals that receive Individualized Home Supports from Enriched Living. These staff are responsible for assisting the individuals we support to complete their daily/weekly goals and provide supports that help them remain in community housing.

Similar to the following positions: Direct Support Professional, ILS Specialist, Positive Support Specialist, Mental Health Practitioner, etc.

The Independent Living Specialist is responsible for:

• Provide Crisis prevention/intervention services to the people served
• Monitor the people supported for symptoms of decompensation, crisis and medication side effects, and intervenes when necessary.
• Incorporate the CSSP and CSSP Addendum into daily interaction with the people served.
• Assist the people supported with problem solving different aspects of independent living
• Maintain complete and proper documentation for direct service hours within 24 hours
• Participate in scheduled and spontaneous recreational events (while still following the Enriched Living Transportation Policy)
• Maintain confidentiality by complying with HIPAA and MN Data Privacy
• Adhere to your weekly schedule, arrive on time, communicate schedule changes, etc.
• Attend mandatory staff meetings, and other meetings requested by the DC/DM
• Maintain clear, open, and honest communication with program management.

APPLICANT REQUIREMENTS:

Applicants must be able to pass a Department of Human Services background check and provide TWO professional references.

Applicants must be oriented to and always work with a person-centered approach.

Applicants must be able to organize and work independently.

Applicants must have strong written and oral communications skills and must be able to document in an objective and effective manner.

Applicants must have a valid driver’s license and have a satisfactory driving record.

Applicants must have a working vehicle with valid auto insurance.

Applicants must be able to lift 40lbs or more.

EDUCATION/EXPERIENCE REQUIREMENTS:

• 1-2 years’ experience working with adults with disabilities, mental illness or chemical dependency OR an associate degree or higher in human services or related field.
• Ability to write reports, program summaries, and other correspondences with support teams.
• Ability to effectively present information and respond to questions from medical professionals, management, staff, people served, and regulatory authorities.
• Ability to read, analyze and interpret general human services periodicals, professional journals, technical procedures, or government regulations.

Job Type: Part-time

Pay: $18.50 per hour

Schedule:

• Monday to Friday
• Overtime
• Weekend availability

Experience:

• Independent Skills Teaching: 1 year (Preferred)

License/Certification:

• Driver's License (Required)
• CPR Certification (Preferred)
