Why does the job exist?

This position contributes to the overall goals of the HIV Services program by coordinating linkage to care and Data-to-Care (D2C) activities to improve results for the state's HIV Care Continuum. It also provides assistance with program administrative functions such as tracking and analyzing client statistics and data, to include reporting to the HIV Services Program Manager and Clinical Quality Management (CQM) team.

How does it get done?

1. Develop, coordinate and implement evidence-based strategies to improve Data-to-Care and retention in care for persons living with HIV, with a focus on but not limited to clients eligible for Ryan White services.
2. Provide Linkage-to-Care support services to persons living with HIV who wish to enroll or re-enroll in services including HIV medical care, as well as individuals moving to the state or transferring services to new providers.
3. Perform additional program and data activities, including but not limited to, (a) Supporting the AIDS Drug Assistance Program (ADAP) Coordinator in ensuring linkage to health insurance and HIV medications, and (b) serving as a liaison to HIV Service Provider (HSP) contract agencies including reviewing client enrollments and invoicing.

1) HIV Service Provider (HSP) contract agency staff including case managers and medical providers.
2) Persons living with HIV (PLWH) in New Mexico

Ideal Candidate

  • Three years minimum experience working in health or public health fields, particularly related to infectious diseases.
  • Excellent communication skills, with bilingual preferred (Spanish, American Indian languages, etc)
  • Experience working with marginalized and underserved populations and communities

Minimum Qualification

Bachelor's degree in Public Health or Education and two (2) years of experience in a health related field. Substitutions Apply. See Substitution Table below.

Employment Requirements

Must possess and maintain a valid Driver's License. Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive Defensive Driving Course Certification as a condition of continued employment.

Working Conditions

State of New Mexico employees may be impacted by the Public Health Order dated December 19, 2022, which requires facilities licensed or certified by the Centers for Medicare and Medicaid Services ("CMS"), including all hospital types, long-term care facilities, nursing homes, hospice facilities and rehabilitation facilities, as well as assisted living facilities and adult day care settings, to adhere to all COVID-related requirements prescribed by CMS, including, but not limited to masking and staff vaccination.

Work is performed in an office setting with exposure to Visual/Video Display Terminal (VDT) and extensive personal computer and phone usage. Some travel may be required.
