Your Specific Responsibilities:

Eligibility screening: AIDS Drug Assistance Program (ADAP) and Ryan White (RW) CORE eligibility.

Enroll and recertify clients in ADAP: insurance availability & coverage; prescription(s) medication dispenses; labs; in ADAP Provide database.

Third party-payer screening. Coordinate with client care managers assisting client access with supporting resources: Medicaid, Medicare, Tricare, VA, employer sponsored insurance, ACA Marketplace, & Pharmaceutical Assistance Programs.

Referral and follow-up with dispensing pharmacies: monitoring client prescription pick up history.

Monitor client lab results for suppression of viral loads.

Discuss adherence measures with clients specific to dispensing of medications and unsuppressed HIV viral loads.

Maintain accurate and up to date electronic client files (Provide Enterprise) and link with the CAREWare system. Conduct regular quality assurance activities including client file reviews, and monitoring client adherence.

Develop working knowledge of Provide Enterprise reports for the preparation of daily client management.

Remain current with changes and usage of ADAP and Ryan White policies and procedures.

Participate in ADAP, HIV treatment adherence, HIV/AIDS education/prevention and care management trainings and webinars.

Complete HIV 500 and 501 courses and maintains 501 updates annually. Provide HIV pre and post-test counseling (anonymous and confidential testing).

Participate in HIV health education/prevention activities with local community.

Required Knowledge, Skills, and Abilities:

Ability to work with diverse population. Knowledge of and experience with computers and computer software. Knowledge of Windows (MS Word, Excel, and PowerPoint) and Outlook. Knowledge of and experience with methods of data evaluation. Ability to utilize problem solving techniques. Ability to communicate effectively both verbally and in writing. Ability to establish and maintain effective working relationships with others. Ability to work independently. Ability to maintain client confidentiality.


Qualifications:

Minimum

Associates degree

Experience in interviewing, counseling, or customer services

Comfortable working with HIV+ clients.

Able to work cooperatively with healthcare professionals to meet client adherence goals.

Willing to work before, during and/or beyond normal work hours or days in the event of an emergency. This includes emergency duties including, but not limited to, response to threats involving any disaster, man-made or natural.

Preferred

Have experience working with a caseload of electronic client or patient files.

Knowledge of Microsoft Office; specifically Excel and Outlook.

Have at least 1 year experience supporting clients with health insurance coverage options.

In addition to English, be proficient in speaking, writing, and reading Spanish or Creole.
