Immediate Opportunity for an LPN
Rapidly growing Senior Home Care Company is looking for an enthusiastic and knowledgeable LPNs to join their all-star team.
What you will be doing:
- Oversee cases for 70-80 patients
- Provide guidance and support to primary family caregivers
- Monitor and provide ongoing assessments and daily progress notes
- Periodic travel required
Qualifications:
- Excellent communication, computer, and organizational skills
- Knowledge of A&D Waivers
- Home and community-based resources is a plus
- Knowledge of community resources related to medical issues, housing issues, and financial and social issues for dealing with clients' care
Job Type: Full-time
Pay: $75,000.00 - $80,000.00 per year
