SUMMARY OF DUTIES:

An employee in this position ensures proper resident care is provided by the direct care partners to residents in a Home for Aged (HFA) Independent Living and Assisted Living Home and is responsible for monitoring each resident’s individualized service plan. Employees of this job work under the supervision of the Director of Resident Care. This employee is a working supervisor and responsible for his/her shift duties and the supervision of the Direct Care Partner(s) during his/her shift.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. Monitor each resident’s service plan to ensure the individuals physical, mental and social needs of the service plan are being met, including reassessment of any resident who has experienced a change in the physical and or mental condition.

2. Direct and monitor resident care provided by the direct care partner to ensure each resident’s individual needs are consistently met each resident’s service plan is being followed to provide the best possible care and living environment for the resident.

3. Communicating new programs, procedures, interventions and/or changes in a resident’s health to the direct care partner(s), and resident’s physician when indicated which includes coordinating necessary resident physician appointments.

4. Assist residents direct care partner in completion of their job responsibilities if necessary; lead direct care partner in gaining exceptional skills in the care of residents; monitor resident direct care partners work performance; regular monitoring of daily documentation.

5. Develop and maintain an open relationship with the residents of the community through regular interaction, encouragement and support residents with their involvement in the community.

6. Function as contact person for residents, resident’s family, physician, healthcare agencies providing additional services to the resident, and community’s contracted pharmacy regarding resident medications.

7. Responsible for coordination of residents’ medications including, medication documentation, controlled substance medication count and disposal according to state regulations; communicate with resident, resident’s family, physician and pharmacy regarding medication orders and or changes in resident medications.

8. Serve as the first line for mitigating concerns with the resident and or resident’s family and provide support and information to each resident and their family; respond promptly to a resident’s need or concern.

9. Meet with the Assistant Director of Resident Care and or Director of Resident Care to determine needs and challenges of resident care, including possible discharge; coordinate with Director of Care to confer with resident and or resident’s family regarding changes in a resident’s status.

10. Monitor documentation in and completion of resident records, medication administration record, and physician orders to ensure compliance; conduct resident chart audits to ensure accuracy and completeness of resident’s medical record.

11. Responsible for the safety and wellbeing of the residents including completion of resident incident reports in compliance with licensing regulations; responsible for safety of residents in case of emergencies; identify potential safety hazards, initiate corrective action to address potential hazard and inform Assistant Director of Resident Care and or Director of Resident Care.

12. Oversee completion of regularly scheduled events regarding resident safety/health, e.g. resident vital signs, fire drills, resident TB testing, flu/pneumonia vaccinations.

13. Maintain clean and organized Medication Rooms, including maintenance of equipment and inventory of resident care supplies including PPE and first aid supplies, ect; report to Assistant Director of Resident Care and or Director of Resident Care needed supplies equipment repairs, ect.

14. Practice appropriate lifting and transfer techniques when assisting residents; awareness of general safety, environmental, and infection control procedures; familiar with (PPE) Personal Protective Equipment and universal precautions related to OSHA – Blood Borne Pathogen Standard.

15. Knowledge of location of community disaster manual and Safety Data Sheets (SDS) and reporting system specified in the Right to Know Policy.

16. Excellent attendance, timeliness in reporting to work, and discussion of all absences, injuries and illness with Administrator; attendance at professional seminars and community activities.

17. Attend minimum of 10 in-service training programs per 12-month period.

18. Perform other related duties as assigned.

SUPERVISORY RESPONSIBLITIES

1. Provide supervision, orientation and training to direct care partners, direct, monitor and motivate staff and staff performance, assess workload, assign work responsibilities.

2. Encourage and provide opportunities for resident direct care partners to provide input into policies and procedures to ensure quality resident services; meet with staff to determine needs and challenges of job responsibilities.

3. Assist the Assistant Director of Resident Care and or Director of Resident Care in the development and implementation of departmental policies, procedures and operations; accountable, dependable and assertive in identifying departmental and staffing needs.

4. Assess problem situations to identify causes, process relevant information, generate possible solutions and make recommendations to resolve problem.

5. Provide leadership and encourage a professional, positive attitude among community staff by being a role model in conduct, appearance, communication, mutual respect and ethical behavior and decision making.

6. Present an open, positive, and professional image to all residents, visitors, co-workers and public by displaying general warmth and kindness in a friendly, courteous, respectful manner at all times and under all circumstances.

7. Regular communication with Assistant Director of Resident Care and or Director of Resident Care and provide input into community’s resident care program.

8. Assist the Assistant Director of Resident Care and or Director of Resident Care with selection of resident Direct Care Partners and make recommendations regarding disciplinary action, work performance, salary adjustments, promotions and terminations or direct care partners.

9. Participate in planning and implementation of ongoing assurance process and improvement efforts consistent with community’s philosophy and mission statement.

POSITION REQUIREMENTS – KNOWLEDGE, SKILLS, ABILITIES

1. Ability to perform necessary job responsibilities of Direct Care Partner and Medication Passer.

2. Friendly, courteous manner; highly motivated, patient; excellent communication skills to direct resident care to range of residents with varying degrees of abilities.

3. Knowledge of assisted living and memory care principles, techniques, methods and current trends/developments regarding resident care including knowledge of Right’s Rights, federal/state licensing and regulations for Home for the Aged community.

4. Knowledge of computer PC operations and software e.g. Microsoft Word, Excel, etc.; good organizational and prioritization skills.

5. Ability to be flexible, solve problems/conflicts effectively in a sensitive and calm manner, act independently and carry out oral and/or written instructions; ability to react quickly and decisively in the event of an emergency.

6. Affinity for seniors and genuine care regarding their well-being with an ability to maintain appropriate attitude and conduct for the welfare of residents; compassionate; demonstrate an appreciation of the heritage, values, and wisdom of the resident and an understanding of their rights, the aging process and diseases of the elderly.

7. Display honesty, integrity, dependability and ability to work as a team member, possess good leadership and interpersonal skills and maintain a positive attitude toward employment; able to communicate effectively and maintain confidentiality of resident and employee information in compliance with HIPPA regulations.

8. Reliable transportation to work with flexibility to work any day of the week, any designated shift.

9. Ability to identify own personal strengths and weaknesses and discuss with supervision.

10. Satisfactory references, background check, drug screen, pre-employment physical and a TB Test for tuberculosis.

EDUCATION & EXPERIENCE

- High school or GED equivalent

- CNA Certification and/or minimum 4 years in health related elder care environment.

- Previous work experience as a Direct Care Partner and a Medication Passer

PHYSICAL REQUIREMENTS AND WORK ENVIRONMENT

1. Work is performed in a residential community setting; possible exposure to unpleasant conditions such as cleaning of human waste and bodily fluids to maintain a sanitary environment for residents, visitors and staff of the community.

2. Speaking and hearing required to perform job responsibilities; reading/writing; understand directions and communicate verbally.

3. Specific vision abilities required include color, close and far vision acuity, depth perception and ability to adjust vision focus.

4. Employee must be in good physical health, and have the ability to climb, balance, kneel, crouch, reach, walk, sit, lift, grasp, perform repetitive motions involving bending, twisting, stooping, pushing, pulling, fingering, wrist motions and foot operations; ability to stand for an extended period of time without significant rest period; possess the strength/stamina to lift more than 50lbs with assist.
