Field Based HSS Coordinator
will be the primary care manager for members with complex medical or behavioral needs who aren’t otherwise managed by the team’s clinicians. The Field Based HSS Coordinator works assertively to locate and engage members in the community. They will develop individualized plans of care that focus on the member’s definition of success. The Field Based HSS Coordinator identifies and overcome barriers to goal achievement, including SDOH barriers. This position will work in collaboration with the interdisciplinary clinical care management team (Pod) to advance interventions aligned with a member’s readiness to change. The Field Based HSS Coordinator will approach their member work with an understanding of how inequities drive health disparities. The Field Based HSS Coordinator will promote health equity.


Schedule
: Monday through Friday 8:00 am to 5 pm Arizona Time Zone. This position is a field-based position with a home-based office. You will work from home when not in the field.


Location:
Phoenix, Tucson (Maricopa or Pima County) and surrounding communities in Arizona


Primary Responsibilities:


  • Supports Pod huddles

  • Identified SME to support CHW in assessment and Plan of Care questions

  • Act as a senior SME for non-RN level activities

  • First point of contact for questions for CHW within Pod

  • Supports RN to lead Pod rounds - taking lead on completing round template

  • Analyzes and investigates QOC issues identified within Pod

  • Provides explanations and interpretations within area of expertise

  • Uses pertinent data and facts to identify and solve a range of problems within area of expertise

  • Investigates non-standard requests and problems, with some assistance from others

  • Prioritizes and organizes own work to meet deadlines

  • Provides explanations and information to others on topics within area of expertise

  • Support the additional training and coaching needed for CHWs

  • Support GL24 FTEs to conduct more field-based care interventions

  • Support documentation supervision and auditing processes

  • Expected to carry similar case load, in addition to these expectations/ Carries an assigned dedicated caseload

  • Uses pertinent data and facts to identify and solve a range of problems within area of expertise

  • Investigates non-standard requests and problems, with some assistance from others

  • Works exclusively within a specific knowledge area

  • Prioritizes and organizes own work to meet deadlines

  • Provides explanations and information to others on topics within area of expertise. - Analyzes and investigates

Required Qualifications:


  • Bachelor’s degree (medical or Behavioral degree) (or higher) OR 2+ years LPN experience OR 4+ years of behavioral health experience

  • 3+ years of experience with behavioral/ medical health

  • Intermediate level of proficiency with Microsoft Office (PowerPoint, Word, Excel, & Outlook) including the ability to edit, create, save, and send spreadsheets and documents and ability to operate and integrate those systems

  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)

  • Access to approved high-speed internet connection or leverage an existing high-speed internet

  • Ability to travel locally to Maricopa or Pima County and surrounding communities in Arizona, approximately 60% of the time and up to 100 miles round trip with Reliable transportation, that will enable you to travel to client and/or patient sites within a designated area, with a current and non-restricted state of Arizona Driver’s License and State-required insurance

  • Reside in the State of Arizona and at a commutable distance to Phoenix, Tucson (Maricopa or Pima County) and surrounding communities in Arizona

  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. Preferred Qualifications:


  • CHW certification

  • Experience in a behavioral health setting (inpatient, outpatient, residential, community mental health, etc.)

  • Experience working in Medicaid and Medicare health care and insurance industry, including regulatory and compliance requirements


Soft Skills:


  • Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others

  • Demonstrated ability to assist with focusing activities toward a strategic direction as well as develop tactical plans, drive performance and achieve targets

  • Problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action
