Sparkles Home Health Care is seeking a reliable and consistent full time and parttime ILS/IHS to work with clients in the comfort of their home and community. You will help individuals become integrated and engaged in the community while fostering the development of independent living skills. This position provides direct supervision to clients in their own homes or a community setting.

Individualized Home Supports uses a person-centered approach to support what is important to and for a person. The service combines both training and supports into a single, flexible service. Individualized Home Support is designed to provide in-home assistance to people who live independently in their own home. Individuals who are receiving support by Sparkles IHS program receive supports based on their level of need.

Individuals may need support ranging from 4 hours per week to as many as 40+ hours per week. IHS staff work collaboratively with other support resources to understand specific supports needed and to develop a Person-Centered plan to assure the individual lives a healthy, involved, and fully integrated life. DSP/IHS/ILS are NOT working in group home setting.

Duties include:

1. You will be providing direct one on one supervision, cueing, maintenance, guidance, instruction, incidental assistance with activities of daily living or assistance with coordination of community living activities to help the person live independently.

2. Implement positive behavior support plans.

3. Help client with organization skills and other goals their want to achieve.

4. Promote client growth through achieving goals identified in their support plans including positive behavior support, vocational, communication, community integration, and other goals as specified.

Accountabilities

• Maintain an up-to-date and comprehensive knowledge on all related policies and procedures, rules and regulations for related to Maltreatment of Minors Act, Vulnerable
• Adult Act, Data Privacy, Documentation, Individual Rights, Bloodborne Pathogens and, Fraud Prevention.
• Manage and maintain all required documentation. Prepare weekly support plan.
• Responding to and reporting incidents as required for HCBS. Following safety practices established by SPARKLES.
• Demonstrate discretion, judgment, and initiative regarding problems and inquiries.
• Maintain confidentiality of client information. Follow established policies and procedures and do not use or disclose protected health information other than as permitted or required by the Agreement or as required by law.

Requirements

• 18 years or older.
• Have a reliable source of transportation to and from the client's home.
• High School Diploma
• Excellent verbal and written communication skills.
• Ability to react quickly, under high pressure with minimal supervision.
• Schedule Flexibility
• Have A Minnesota Driver’s License, Full Coverage Insurance, and Reliable Vehicle.
• Be willing to transport clients in your vehicle
• This position involves driving your own vehicle and clients.
• * Staff must be COVID-19 vaccinated AND willing to provide proof of vaccination

Mandatory Requirements of the position

• DHS Background Clearance
• Reliable insured vehicle
• Clean driving record

Pay: $ 17.00 - $19.00 per hour

• Day, Afternoon and Evening to fit your schedule
