Coordinator of Care – Hospice/Home Care - Days

Under the supervision of the LCHS Director, the Care of Coordinator is responsible for coordinating and integrating high quality, cost effective care delivered to patients across an episode of illness or throughout the entire continuum of health care delivery. Assesses patient’s/family’s/caregiver’s ability to understand patient’s post-acute care needs, assisting with Homecare services or Hospice services as necessary after hospitalization or Skilled nursing facility stay. Collaborates with the social workers / case management departments at different facilities and hospitals throughout Westchester County in the assessment of the patient/family post discharge continuing care needs in the community.

This position requires an RN experienced and knowledgeable in Certified Home Hospice care and Certified Home Health Care. Also, an understanding of the Medicare hospice benefit, hospice diagnoses, and end of life disease processes is essential. You’ll be a frontline contact for patients, families, physicians, and hospitals of Jansen Hospice & Palliative Care and Lawrence Homecare of Westchester referrals. Responsible for coordinating patient services and communicating with colleagues in the field. This is a full-time day shift opportunity Monday - Friday from 10:00am – 6:00pm or 7:00am – 3:00pm. This position supports both Jansen Hospice, Palliative care and Lawrence Homecare of Westchester a Certified Home Care Agency..

Preferred Criteria

• Two years case management in an inpatient, HMO, insurance or other outpatient setting
• Ability to work as strong, committed member of our interdisciplinary team of professionals

Required Criteria

• 3-5 years of health care experience in an acute care setting
• Current New York State RN license
• BCLS certification (American Heart Association)
• Ability to function as core member of an interdisciplinary team
• Excellent communication skills and computer proficiency required.
