Minimum Qualifications-

Level II- 3 years' experience required or as follow:

For Appointment to Level I
1. One year of full-time experience in a government agency or community organization in providing community services to the public or in assisting members of the community in obtaining community services; or
2. Completion of an approved six-month training program of combined classroom training and on-the-job experience in community liaison work, plus six months of full-time experience as described in 1 above.

For Appointment to Level II
In addition to meeting the qualification requirements of Assignment Level I, appointment to Level II requires:
1. Two additional years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or
2. Education and/or experience equivalent to 1 above. Study at an accredited college in sociology, psychology or other behavioral science may be substituted on the basis of 30 semester credits for each year of the experience described above. However, all persons must have at least one year of the full-time experience described above.

Job Description

Summary of Duties and Responsibilities:

Under the general direction of the Ambulatory Care team and in close collaboration with the Program Manager in the Office of Ambulatory Care and Population Health and as part of the NYC H+H Public Health Corps, supervises a team of up to 10 Community Health Workers (CHWs) performing patient-centered, community health work with patients, their parents and caretakers in collaboration with interdisciplinary teams at multiple clinic sites in NYC H+H primary and specialty care clinics, reporting to Pediatrics Primary or Specialty Care Leadership at each site. Supervisors are assigned a team of CHWs, trained frontline public health workers who are trusted members of and/or have a thorough understanding of the communities they serve. Serves a critical role in improving quality of care for our System’s patients through oversight of community outreach activities aimed at connecting patients and their families with available resources including community and parenting services, mental health support and/or prenatal care to support their health goals. All references to patient below may include their parents and/or caretakers, where appropriate.

General tasks and responsibilities will include:

  • Partner with Program Manager and clinical leadership to refine and deliver CHW Program to support specific facility, department and patient needs
  • Participate in regular meetings with Program Manager and clinical leadership to review program progress
  • Assist in recruitment and hiring of CHW candidates, as needed
  • Lead training and mentoring process for CHWs, including the development of individualized learning plans to meet the specific professional development needs of a workforce with diverse backgrounds
  • Supervise CHWs based at one or more clinical sites serving hundreds of high-need patients per year, by providing support, coaching, and facilitating problem-solving for CHWs at home or community-based sites, as needed
  • Conduct weekly 1:1 Supervision meetings with each CHW to review patient caseload and provide feedback and recommendations on intervention approach
  • Escalate issues to Program Manager or Clinical Leadership as appropriate
  • Facilitate weekly case conferences with the CHW team to review goal-directed interventions and work with
  • CHW team to develop and refine patient recruitment strategies to ensure enrollment targets are met
  • Develop channels to facilitate communication between CHWs and clinic and hospital care teams; educate clinical teams on CHW program
  • Manage patient outreach list and track enrollment progress using the electronic health record (Epic) and other programs, as needed
  • Oversee CHW patient intake process aimed at developing patient-centered, goal-directed treatment plans
  • Stay up-to-date on, and ensure CHW team is informed of, evolving patient resources including hospital resources, community services, and benefit programs
  • Monitor and track program progress and effectiveness of CHWs
  • Identify and carry out CHW program quality improvement initiatives such as direct CHW observation and patient follow-up interviews
  • Participate in ongoing training activities related to the community health worker role and program, as well as public health emergency response skills relevant for the Public Health Corps
  • Perform other related program tasks, as needed and perform duties in response to public health emergencies, when necessary
