Position Summary

Assisting and reporting to the Mobile Clinic Manager, the Community Engagement Liaison is responsible for facilitating the Community Care Connection mission to provide high quality health screenings and education to underrepresented community groups and patient populations. This role is a unique fusion of community outreach, patient engagement and assistance with resource needs. The Community Liaison acts as a bridge to advocate between patients, community members and Rosalind Franklin University Health Clinics. In this role you will engage in telephonic and face to face interactions with patients in the community to identify barriers, problem solve and facilitate patient access to community resources.

Rosalind Franklin University Health Clinics' mobile health vehicle embodies the university's commitment to health equity and access to compassionate, quality care. The Community Care Connection (CCC) travels throughout Lake County to reach underserved populations and offer treatment, prevention and education. The CCC also serves as a powerful educational environment for RFU students who volunteer to assist the licensed physician assistants (PAs) and nurse practitioners who staff the mobile unit.

Essential Duties & Responsibilities:

  • Advocate for diverse underrepresented patients and community groups by developing and maintaining strong relationships with respective populations and community programs
  • Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement.
  • Collaborate with the mobile clinic team and Athena Healthcare System to improve community outreach which includes tracking patient barriers and data collection, following trends reflected by the community to effectively assist in healthcare needs.
  • Follow – up with patients via phone calls, home visits and visits to other settings where patients can be found.
  • Maintain an up to date list of community resources, contact persons and programs that can be shared with community members that access the Community Care Connection mobile clinic.
  • Assist with clerical duties such as making copies or gathering supplies for educational sessions.
  • Share with community members and community programs information about the CCC via flyer sharing and/or participating in community events.

Conditions of Employment

  • Must achieve satisfactory results from a background check and drug screen
  • Must have proof of COVID 19 vaccination and booster or a university approved exemption

Required Education & Experience

  • High school diploma or equivalent

Required Knowledge, Skills, & Abilities

  • Interpersonal skills to engage with diverse and underrepresented community groups and patient populations; a strong passion for leadership
  • Ability to motivate and coach patients in healthcare literacy
  • Knowledge of local and community health resources
  • Ability to work independently and be high functioning in a team environment to manage patient and community relationships
  • Excellent oral and written communication skills
  • Basic computer skills/data entry; MS Office programs (Word, Outlook and Excel)

Preferred Qualifications

  • 1 year community outreach experience
  • Community Health Worker certification
  • Bilingual (Spanish/English)
