Palliative care is specialized medical care for people with serious illnesses, which strives to provide patients with relief from the symptoms, pain and stress of their condition. In Palliative Medicine we utilize a multidisciplinary approach, relying on input from professionals across the continuum of treatment to formulate a plan of care that can relieve suffering in as many areas of a patient's life as possible. The Palliative care team aims to address the full spectrum of physical, emotional, spiritual and social concerns that may arise with advanced illness.

As the Care Management Services Registered Nurse, you will provide care management and coordination for patients enrolled in the Care Management Program in the Primary Care setting, in partnership with the primary care practice health. You will serve in an expanded health care role to collaborate with specialists, members of the health care team and patients, families and caregivers, to ensure the delivery of quality, efficient and cost effective health care services. We are looking for someone dynamic and dedicated, with great organizational and communication skills and a commitment to creating a healthier Hawai‘i.

Minimum Qualifications: Must meet education requirements for Hawai‘i state licensing. Current Hawai‘i RN license. Bachelor’s Degree in Nursing or related field. Current American Heart Association Health Care Provider Basic Life Support (BLS) CPR card. Current driver’s license and valid Hawai‘i auto insurance with short notice access to an automobile to conduct site visits, as required. Two (2) years of clinic experience in a Primary Care setting. Experience working within a collaborative interdisciplinary team.

Preferred Qualifications: Certification in Case Management. Experience in care coordination for patients. Care management experience. Experience as participant in continuous quality improvement. Working knowledge of EMR (Electronic Medical Record).
