Organization Overview and Functions

Wasatch Homeless Health Care, Inc. (WHHC) dba Fourth Street Clinic (FSC), Salt Lake City’s leading nonprofit health care provider for homeless Utahns operates with a core staff of over 80 and a volunteer network of more than 150 individuals. Annually, the health center provides services to over 5,000 men, women and children experiencing homelessness through approximately 21,000 visits through medical services, behavioral health services, dental services, case management and health education classes. Fourth Street Clinic has an onsite pharmacy that provides over 60,000 prescriptions annually.

Fourth Street Clinic is seeking a licensed therapist (LCSW or CMHC) or MSW/ACMHC working on licensure (supervision will be provided) who is interested in working in a non-traditional, progressive behavioral health program. FSC’s behavioral health services focus on screening, assessments, individual and group counseling, and medication management. Therapists act as treatment providers and advocates, helping clients gain access to resources while addressing their behavioral health needs, including mental illness and substance use disorders. This position will be co-located at Magnolia Apartments, a permanent supportive housing program which houses individuals who have experienced chronic homelessness. This role will work closely with FSC’s Medical Case Manager, our integrated health care team, partnering teams from The Road Home, other partner agencies, and behavioral healthcare services in Salt Lake County.

Duties & Responsibilities

The following information is intended to be representative of the essential functions performed by incumbents in this position and is not all-inclusive.

  • Serve as a member of an integrated team and work to ensure patients have access to whole person care, including medical, pharmaceutical, and dental services.
  • Provide behavioral health consultation to co-located FSC staff and partner agency teams.
  • Engage in assertive outreach to tenants of Magnolia. Find creative ways to build rapport and implement therapeutic interventions.
  • For clients engaged in a longer-term therapeutic relationship, perform ASAM and biopsychosocial assessments to inform treatment planning and referrals to outside entities for higher levels of care when appropriate.
  • Work with clients that present in crisis by providing crisis management, assessing safety, and creating safety plans.
  • For clients on caseload who are also FSC patients, assist the primary care team in clinic in developing care management processes; such as: the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease.
  • Provide individual and group therapy for patients with mental health and substance use disorders.
  • Provide assessment, consultation, and brief intervention for tenants with mental health concerns.
  • Formulate appropriate mental health diagnoses.
  • Assure quality of care by adhering to evidence based practice, therapeutic standards, and standards of care set by DOPL.
  • Maintain accurate and timely documentation of all clinical encounters in the clinic’s electronic health record. Record social determinants of health data and work to improve these outcomes for each client on caseload.
  • Follow HIPAA and 42 CFR confidentiality guidelines.
  • Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional associations.

Necessary skills/attributes

  • Competency in multiple psychotherapy modalities and evidence based theoretical frameworks including but not limited to: harm reduction, trauma informed care (TIC), cognitive behavioral therapy (CBT), seeking safety, acceptance and commitment therapy (ACT), motivational interviewing, and medication assisted treatment (MAT).
  • Ethical and professional behavior.
  • Demonstrate excellent communication skills.
  • Desire to work with diverse and vulnerable populations.
  • Interact and collaborate with professionals from many different social service and government agencies.
  • Demonstrate emotional resilience and stability.
  • Ability to work quickly when needed and flexibility with scheduling, must be able to remain calm in tense and/or chaotic situations.

Qualifications

  • Graduate of an accredited master’s program in Social Work or Mental Health Counseling, or equivalent degree, and licensed in the State of Utah.
  • BLS Certified within 90 days of hire.
  • Bilingual English/Spanish desirable.
  • Must be able to pass a background check.

Job Type: Full-time

Pay: $63,500.00 - $72,000.00 per year
